About The Position

We are looking for a Clinical Social Worker for Behavioral Health in Albany, OR. This role focuses on Outpatient Hospice/Medical Social Work, assisting residents in assisted living facilities and adult foster homes. Services include intermittent skilled nursing visits, 24-hour on-call nursing services, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support. Medications and equipment are included as appropriate.

Requirements

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required.
• Healthcare Provider Level BLS required.
• Must use personal automobile for services.
• Must have a valid driver's license.
• Must have auto liability/property damage insurance as required by law.
• Must maintain vehicle in good working order.
• Minimum 1 year of experience as a social worker.

Nice To Haves

• Oregon LCSW (Licensed Clinical Social Worker) preferred.
• Experience as a social worker in a hospital or medical setting preferred.

Responsibilities
