Requirements

• State License: Hold a clinical Social Worker license from any US State
• Experience: A minimum of two years’ full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.
• Education: Whether you hold a Doctorate or Master's degree, your expertise is valued here.
