About The Position

Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services. The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliant Documentation Specialists and skilled administrative staff. Case Management in the Hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along the continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient’s right to self-determination. This role provides clinical social work services to address the psychosocial needs of patients and families. It involves individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. The role collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Requirements

• Master's Degree in Social Work
• Licensed Clinical Social Worker (LCSW)
• No Experience
• No Experience (Supervisor Experience)

Nice To Haves

• Medical Social Work experience highly preferred
• 2-5 years (Experience)

Responsibilities

• Conducts screening and assessment activities to meet the needs of the discharged patient.
• Applies assessment findings to prepare a patient-centered plan of care.
• Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
• Assessment; crisis intervention and resource provision.
• Discharge planning.
