About The Position

Royal Oak Hospital, recognized by U.S. News & World Report as the #2 hospital in Michigan and one of the top hospitals in the country, serves as a major academic and referral center. It holds Level I adult and Level II pediatric trauma designations and has achieved Magnet designation four consecutive times. The hospital is also home to Michigan’s first Diagnostic Imaging Center of Excellence. The Clinical Social Worker MSW position involves working collaboratively within an interdisciplinary team to support patients and their families facing complex psychosocial needs. This includes a range of interventions such as coordinating alternate sites of care, discharge planning, addressing instances of abuse or neglect, facilitating adoptions, managing substance use disorders, assisting with end-of-life decisions, providing appropriate referral resources, offering bereavement support and adjustment counseling, facilitating guardianships, and managing behavioral health concerns, including psychiatric transfers. The role requires conducting thorough psychosocial assessments with patients and/or families to identify needs, especially for those requiring post-discharge care, and developing and executing a corresponding plan. The social worker is responsible for effective and timely communication with alternate care sites and community agencies to best meet patient needs. Documentation of assessments and plans in the electronic medical record must accurately reflect the patient’s condition, situational factors, transition of care needs, and psychosocial imperatives. Additionally, the social worker provides consultation and resources to the healthcare team and is expected to maintain current knowledge of available community resources.

Requirements

  • Master’s degree in social work
  • State of Michigan Social Work License
  • LMSW expected within 5 years of start date

  • Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred

Responsibilities

  • Work collaboratively with members of an interdisciplinary team in order to assist patients and families with complex psychosocial needs.
  • Intervene in areas such as alternate site coordination, discharge planning, addressing all forms of identified abuse/neglect, adoption, substance use disorders, assisting with end-of-life decisions, providing appropriate referral resources, bereavement support, adjustment counseling, facilitating guardianships and behavioral health concerns (including psychiatric transfers).
  • Conduct psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and execute a plan.
  • Communicate with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient’s needs.
  • Complete and document the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, transition of care needs and psychosocial imperatives.
  • Provide consultation and resources to members of the healthcare team.
  • Maintain relevant and current knowledge of community resources.
