About The Position

Under the supervision of the Citywide Focus Kujichagulia Program Director, the incumbent provides continuous, comprehensive mental health services including crisis intervention, outreach, therapy and case management to a caseload of severely mentally ill adults who are high users of psychiatric mental health services, often with concurrent substance abuse and/or health problems. The Kujichagulia team focuses on African American clients. Assesses presenting problems, obtains psychosocial history, conducts mental status examination and diagnoses using the Diagnostic Statistical Manual (DSM-IV); participates in multi-disciplinary treatment team; act as liaison to inpatient units and other service providers to facilitate referrals and service/treatment; maintains client records according to California, City and County requirements; maintains a master-level requirements; attends staff meetings and participates in training session, other duties assigned.

Requirements

Masters degree in social work, psychology, or counseling or related field from an accredited school of social work or equivalent experience/training and knowledge and abilities essential to the successful performance of the duties assigned to the position
A Master's degree in social work, psychology or counseling from an accredited school of social work and possession of the requirements necessary for application for licensure as a clinical social worker, as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
Ability to work cooperatively and communicate effectively with others.
Ability to work in a high stress, high volume area and work well under pressure and with deadlines.
Ability to follow oral and written instructions accurately, and be detailed oriented.
Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes.
Knowledge of medical and/or psychiatric terminology
Excellent communication and interpersonal skills
Able to work as a team member in a culturally diverse setting
Able to lift 50lbs and climb stairs
Demonstrated excellent attendance and reliability
An understanding of mental health issues and sensitivity toward mental health clients
Ability to work with clients and program staff from diverse cultural and social class backgrounds
Eligible to test for licensure with the Board of Behavioral Science as An Associate Clinical Social Worker (ACSW) or Intern Marriage Family Therapist (IMFT).
Two years working with severely mentally ill adults, the homeless and dual diagnosed populations
One year experience working with African American clients
One year experience doing community based case management and the ability to do outreach including to SRO hotels

Responsibilities

Provides ongoing mental health services, including psychotherapy (Individual, Group, & Family), Crisis Intervention, Case Management and Outreach to a caseload of severely mentally ill clients.
Makes case terminations and discharge plans, when clinically appropriate.
Develops a comprehensive individual service plan for each client, integrating knowledge of a wide range of human service resources.
Provides consultation to a multi-disciplinary treatment team on the psychosocial ramifications of psychiatric illness.
Facilitates treatment coordination among various agencies and advocates for and links clients to needed services.
Provides consultation to and maintains liaison with campus and community programs, including educational, legal, and social organizations involved in case management.
Acts as liaison to inpatient units to facilitate referrals and service treatment.
Screens referrals for meeting program criteria; conducts intake of new clients, including assessment, community functioning, and service needs, when appropriate.
This entails assessment of presenting problems, psychosocial history, mental status and diagnosis (DSM-IV).
Responds to unscheduled “walk in” clients, utilizing crisis intervention techniques and providing 5150 services as needed.
Serves as weekly OD screening clients for entry into the clinic.
Maintains client’s records according to California, City and County requirements.
Records data in client’s charts.
Meets program productivity standards.
Assists in processing quality improvement requests.
Completes billing and other documentation of services in a timely fashion.
Attends staff and clinical team meetings, and participates in training sessions (e.g., In services, grand rounds).
Identifies program problems and makes recommendations to supervisory and administrative staff.
Provides education to the community regarding program services, client needs and treatment.
Assists in planning and providing in-service to staff.
Provides orientation and didactic training to interns.
If desired and appropriate, supervises student(s) and participates in student training program meetings.
