About The Position

The Dr. Solomon Carter Fuller Mental Health Center (DSCFMHC) is seeking an experienced and professional Clinical Social Worker C (CSW C). The CSW C will perform biopsychosocial diagnostic assessments by conducting interviews of patients and their families/significant others, contacting outpatient service providers, and investigating histories to identify strengths and needs and develop a treatment plan. The role involves coordinating treatment plans, providing clinical case management, coordinating pre-discharge arrangements, and offering skilled clinical social work services including individual, group, and family therapy. Additionally, the position requires providing psychoeducation, documenting interventions, leading inpatient groups, completing financial assessments, and staying updated on relevant regulations. The role may involve supervision and participation in quality improvement activities.

Requirements

  • Master's degree in social work.
  • Licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work.
  • At least two years of full-time, or equivalent part-time, professional experience as a clinical social worker after earning a Master's degree.
  • Demonstrated knowledge of the principles and practices of psychiatric and basic social work practices, including substance abuse treatment, outpatient care to people with severe mental illness, physiological, psychotherapies, psychological, and emotional growth, and development.
  • Familiarity with the types and applications of emergency medical procedures, such as CPR.
  • Basic uses of diagnostic procedures employed in social work practice.
  • Knowledge of interviewing and problem-solving techniques.
  • Ability to follow and give clear and precise oral and written instructions.
  • Capacity to participate in a multi-disciplinary health care team.
  • Skilled in preparing and implementing treatment plans.
  • Ability to demonstrate the knowledge and skills necessary to provide care to adult and geriatric patients.

Responsibilities

  • Completes a comprehensive written biopsychosocial assessment within five days of patient admission.
  • Coordinates the development of individual treatment plans for patients on caseload.
  • Coordinates Periodic Reviews for assigned cases.
  • Provides clinical case management for assigned cases.
  • Coordinates pre-discharge arrangements and communications.
  • Completes oral and written referrals to aftercare services.
  • Provides skilled clinical social work services to patients and their families utilizing various intervention modalities.
  • Provides highly skilled psychoeducation services to patients, families, and other service providers.
  • Documents interventions in progress notes, group treatment notes, and MHIS assessment documents.
  • Leads and/or co-leads at least one inpatient group per week.
  • Documents discharge plan as a component of weekly progress note.
  • Completes financial assessments.
  • Refers identified needs to financial benefits coordinator.
  • Maintains knowledge of current developments in statutes, case law, and departmental/agency regulations.
  • Reviews forensic caseload weekly.
  • May provide supervision and leadership within the Social Work Department and the facility.
  • Participates in agency and department performance improvement/quality improvement activities.
  • Attends seminars, workshops, conferences, and staff meetings.
