Requirements

  • Five (5) plus years of clinical experience
  • Two (2) plus years of leadership experience in a healthcare setting
  • Strong leadership, coaching, and team development skills
  • Knowledge of population health, quality metrics, and value-based care principles
  • Ability to analyze data and translate findings into actionable improvements
  • Excellent communication, interpersonal, and organizational skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Proficiency in electronic medical records and healthcare systems
  • Proficiency in Microsoft Office Suite
  • Master’s degree in Nursing
  • Current, unrestricted Nurse Practitioner license.
  • Active, unencumbered FNP license in Arizona and California (or willing to obtain within 30 days of hire).
  • Board certification (e.g., ANCC, AANP).
  • Willingness to obtain additional state licensure as needed.

Responsibilities

  • Provide clinical direction and support to APPs across clinical services programs.
  • Develop and implement clinical protocols, policies, and workflows.
  • Serve as a resource for complex, urgent, and emergent clinical situations.
  • Lead, manage, and mentor a team of APPs.
  • Oversee recruitment, onboarding, training, and ongoing professional development.
  • Conduct performance evaluations, assess competencies, and implement corrective action plans as needed.
  • Conduct chart reviews to identify documentation gaps and opportunities for improvement.
  • Lead initiatives to improve documentation accuracy, coding integrity, and quality gap closure.
  • Develop and implement risk adjustment improvement strategies and remediation plans.
  • Provide education and training on clinical documentation and best practices.
  • Establish strategic goals aligned with organizational priorities.
  • Monitor program performance and implement improvement strategies.
  • Coordinate with cross-functional teams to ensure appropriate resources, equipment, and operational support.
  • Build and maintain relationships with internal teams, providers, and external stakeholders.
  • Collaborate with healthcare providers to ensure coordinated, high-quality care.
  • Participate in performance and operational meetings with leadership and partners.
  • Support management of program performance in alignment with scope of work (SOW) and regulatory requirements.
  • Monitor performance trends and contribute to action plans addressing gaps or underperformance.
  • Prepare and present clinical, quality, and operational outcomes to internal stakeholders.
  • Ensure compliance with all HIPAA regulations and protection of PHI.
  • Maintain and enhance clinical knowledge through continuing education and professional development.
  • Participate in quality assurance and process improvement initiatives.
  • Travel up to 25% based on business needs.
  • Perform other duties as assigned.
