Under the direction of the Program Manager, the Clinical Services Case Manager plays a key role in supporting the coordination and delivery of case management services within the clinical services team. This position is responsible for managing incoming referrals, maintaining waitlists, facilitating triage processes, and ensuring clients are connected to appropriate services in a timely, equitable, and client-centered manner. The Case Manager works collaboratively with internal multidisciplinary teams, external partners, and First Nation communities to support seamless service navigation while maintaining accurate documentation and adherence to organizational policies and procedures.
