About The Position

EMPACT- Suicide Prevention Center, part of the La Frontera family, is seeking Clinical Service Liaisons to join their Adult Services team in Tempe or Glendale offices. This role involves providing direct services, managing a caseload of adults receiving medication-only services, conducting comprehensive intake assessments, and participating in special projects within the Adult Program.

Requirements

  • Bachelor's Degree in Behavioral Health related field preferred or meet BHT requirements.
  • Behavioral health experience.
  • Ability to be flexible and problem-solve.
  • Ability to use creativity in service delivery approach.
  • Must have Fingerprint clearance card or be eligible to obtain one.
  • Must have a valid AZ driver's license with no major infractions in the past three years.
  • Must have regular access to reliable transportation.
  • Must be 21 years of age or older at time of employment.
  • Demonstrated ability to remain calm in crisis situations.
  • Demonstrated ability to work independently and cooperatively.
  • Demonstrated ability to recognize personal limitations.
  • Demonstrated ability to relate positively and with empathy to all persons regardless of age, race, creed, gender or sexual orientation.

  • If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required.
  • At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
  • Staff members who provide direct client services must be at least 21 years old.
  • All employees will have a minimum of a high school diploma or equivalent.

Responsibilities

  • Provide initial intakes for adult clients, including comprehensive assessment, diagnosis, needs evaluation, and referral.
  • Maintain a caseload of adult medication-only clients, conducting Adult Clinical Team (A.C.T.) meetings for documentation and needs assessment.
  • Assist with the Transition to Adult process for children aging out of Children’s Services, ensuring thorough documentation for a smooth transition.
  • Complete SMI Determination paperwork in collaboration with the prescriber.
  • Include the adult’s family and/or support system in services whenever possible.
  • Deliver 40 hours of work per week.
  • Provide case management services to support medication services as needed.
  • Work independently and responsibly, participating in the development, implementation, and evaluation of procedures for organizing, tracking, and maintaining medication-only client records and services.
  • Produce complete, timely, thorough, quality clinical documentation of all services rendered, both billable and non-billable, as required.
  • Ensure quality service delivery, including clinical care and all associated duties.
  • Maintain all client records in up-to-date status, in accordance with agency policies, and RBHA, state, and federal requirements.
  • Participate in the agency’s Quality Management Program as needed or requested.
  • Participate actively in regular supervision, accepting feedback and suggestions collaboratively.
  • Interact regularly and constructively with the clinical team, support staff, and administrative staff.
