The Adult Ambulatory Oncology Clinic at OHSU Center for Health and Healing, building 2 (CHH2) serves patients within Medical Oncology, Hematologic Malignancies, Cellular Therapy, Gynecologic Oncology, Palliative Care and Classical Hematology. We collaborate with Radiation Oncology, Surgical Oncology, Infusion services, Dietitians, Pharmacy, Social Work, Case Management and other members of the multidisciplinary health care team to provide exceptional oncology care for our patients and community. This position works with Hematologic Malignancy and Cellular Therapy patients. The Nurse Navigation team coordinates care for newly referred patients to the Ambulatory Oncology Service Line and serves as the first point of contact for oncology clinical care in the OHSU system. The nurse navigator (NN) thoroughly reviews clinical information (records, diagnostic test results, etc.) submitted with the referral reviewing for urgency, other needed tests, potential eligibility for clinical trial and to identify which physician/s best suited for consult. The NN calls the patient within two business days of referral to assess the patient’s understanding of their condition and to identify physical and psychosocial needs and known or potential barriers to care. Using parameters set by the oncologists, the NN prioritizes the timing of the consult. This early dialogue with the patient and family provides them with information to help them prepare for the dialogue with the oncology provider. The NN summarizes this interaction via a signed navigation assessment note in Epic which serves as a thorough hand off to the physician and clinical team/s that will consult and/or follow the patient. The NN collaborates with the Knight Access Liaisons (med onc) or PAS teams (surg onc) to track that pathology slides/reports and pertinent records, imaging, labs prior to the first consult arrive prior to the first consult. The Nurse Navigator will work collaboratively with the Knight Cancer Institute faculty experts and their interdisciplinary teams, including attending the multi-disciplinary tumor board and the multi-disciplinary lung clinic on campus on Fridays. This position will work with the clinical nurse manager and other ambulatory oncology nurse navigators to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in gathering data for new patients that will allow the team to create a plan of care at the patient’s first visit and, through the work up phase of their disease. The NN attends the specialty tumor board, clinical team meetings, and participates in any multi-disciplinary clinics. The NN may meet with new patients in clinic or on the inpatient unit as determined by the specialty team. The NN may follow the patient through the diagnostic work up and also assist with transitions of care for the patient between treatment modalities. The NN interfaces with research staff, and works to identify patients who may be suitable candidates for clinical trials.
