The Department of Family Medicine at East Portland is a Federal Qualified Health Center (FHQC) providing primary care to a diverse patient population, including pregnant individuals, babies, children, adolescents, adults, and geriatrics. Patients often present with moderately stable to unstable conditions, complex medical issues, and Social Determinants of Health (SDOH) barriers requiring comprehensive medical and nursing interventions. This role involves cross-training to support other primary care clinics within the Department of Family Medicine and the Division of Internal Medicine. Key responsibilities include remote phone triage, managing MyChart messages, reviewing and following up on results, facilitating transitions of care, and other duties to support clinic workflows and the RN team. The services offered by the provider team encompass primary care, behavioral health and counseling, reproductive health, transgender healthcare, prenatal care (including deliveries), the Project Nurture Program, pediatric care, medication-assisted treatment (MAT), minor procedures, and health maintenance with preventive care. Most nursing work is performed outside of scheduled visits and includes nurse telephone triage, in-clinic nurse triage, OB intake and support visits, wound care, medication administration, patient education, transitions of care and care coordination, team-based care and interdisciplinary collaboration, chronic disease management, and foot care clinics.
