About The Position

At ComForCare Home Care, we believe aging in place is a gift—and our mission is to make it possible. We are seeking a compassionate and experienced Registered Nurse (RN) to join our team in a hybrid clinical leadership role. This position combines hands-on field assessments with oversight of care quality, compliance, and clinical support for our caregiving team. This is a highly collaborative role ideal for an RN who enjoys both direct client interaction and guiding the clinical standards that ensure exceptional in-home care. This is a unique opportunity to blend clinical practice with leadership in a non-facility setting. You’ll have the autonomy to work in the field while also helping shape the quality and consistency of care across our agency. You’ll be part of a supportive office team and play a key role in strengthening both caregiver performance and client outcomes.

Requirements

• Current Oklahoma RN license in good standing
• Strong clinical judgment with the ability to balance autonomy and collaboration
• Leadership mindset with excellent communication and coaching skills
• Comfortable working in both field-based and office/oversight capacities
• Reliable transportation and willingness to travel for home visits

Nice To Haves

• Experience in home health, hospice, geriatrics, or similar settings preferred

Responsibilities

• Conduct initial and ongoing in-home client assessments as part of active case management
• Develop, update, and oversee individualized care plans
• Maintain clinical compliance with all state regulations and agency standards
• Provide training, coaching, and clinical guidance to caregivers and field staff
• Support onboarding and orientation of new nursing and caregiving team members
• Assist with quality assurance and ongoing case reviews
• Step in to support field operations and client visits as needed, including managing a defined caseload
