About The Position

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a Clinical Review Intake Team Lead to join their award-winning team. Requirements

  • Minimum 1 year of experience in Intake Coordination.
  • Excellent communication and public speaking skills.
  • Ability to interact professionally with company staff, referral sources, patients, and others.
  • Ability to navigate multiple systems simultaneously.
  • Basic Excel skills are required.

Nice To Haves

  • 1 year of Team Lead experience highly preferred.
  • Intermediate Excel skills preferred.

Responsibilities

  • Perform all duties and responsibilities of the Central Intake Team in accordance with the Department’s Standard Operating Procedures.
  • Monitor and Oversee Central Intake referral requests are processed timely and accurately daily.
  • Collaborate with other Central Operations Teams as well as with Leadership to identify, document, report gaps in process(es) and contribute to propose/create solutions.
  • Track both individual and Team performance metrics daily.
  • Responsible for developing and sustaining a team environment that maintains motivation to meet both individual and Team objectives/goals.
  • Oversee Central Intake processing and ensure Standard Operating Procedures are being followed.
  • Problem Resolution: Responds to Regional Leaders inquiries on potentially delayed referrals with a thoroughly documented timeline and conditions impacting completion.
  • Runs and distributes reports demonstrating team performance daily, weekly and monthly to Supervisor and/or Leadership Team.
  • Demonstrates, supports, and adheres to company values as an intrinsic part of team’s daily collaboration efforts: Beyond Expectation, Attitude, Integrity, Culture of Discipline, Appreciation.
  • Identify trends, opportunities, and report data to direct supervisor same day.
  • Collaborate with Supervisor to develop, implement and revise Standard Operating Procedures to meet company growth and business.
  • Protects the confidentiality of patient information.
  • Oversees orientation and manages IDPs for new members of the team.
  • Contributes to the development of team members through continuous training, coaching and mentoring efforts.
  • Cross-Train and Support other Central Operations Teams with Special Projects.
  • Additional Duties/Tasks may apply as the business needs of the company evolve.
