The Clinical Rehabilitation Specialist will be responsible for the development and implementation of educational and community relations activities that support awareness and utilization of the rehabilitation program's services. The incumbent will also demonstrate accountability for and contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible team fashion. This incumbent will demonstrate knowledge as an Acute Rehabilitation content expert. The role requires the ability to review patient medical charts, understand pharmaceuticals, test results, therapy evaluations, pre-existing conditions, and possess general medical knowledge of the patient. The specialist must be able to clearly and professionally interact with patients, families, and healthcare providers to gather additional clinical information and past history. Key responsibilities include conducting thorough patient assessments to identify potential admissions, completing detailed Pre-Admission Screens according to facility and payer policies, conducting target area research including analysis of like programs and alternative levels of care, scheduling meetings and conferences for medical professionals and referral sources, providing patient updates to physicians, payers, case managers, social workers, and other relevant persons, maintaining strong relationships with referral sources through excellent service, and participating in direct contact with patients and patient representatives for clinical or educational services. The specialist will also develop educational and community relations plans in consultation with the Program Director, Medical Director, and host hospital CEO, and maintain effective communication and listening skills with all stakeholders.
