InterCommunity, Inc. is a Federally Qualified Health Center Look-Alike (FQHC LA) committed to providing accessible, compassionate care to everyone — regardless of life situation or ability to pay. We offer same-day primary care and a wide range of behavioral health services across our community health centers. Our Addiction Services Division provides a full continuum of care, including primary care integration, residential detox and treatment, outpatient mental health and substance use services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services, and social rehabilitation. The Clinical Quality Nurse supports the agency’s Continuous Quality Improvement Program and standards of care determined by regulatory bodies including the Joint Commission, HRSA, DPH, and others. This role supports change implementation across the agency by collaborating with clinical teams, supporting training needs, and Epic Optimization initiatives. With a focus on care improvement, the Clinical Quality Nurse participates in efforts to achieve established performance metrics, develops and analyzes effective reports, and promotes data-driven decision-making to enhance clinical outcomes.
