About The Position

We offer same-day primary care and a wide range of behavioral health services across our community health centers. Our Addiction Services Division provides a full continuum of care, including primary care integration, residential detox and treatment, outpatient mental health and substance use services for adults and children, intensive outpatient programs, employment and community support, mobile crisis evaluations, judicial support services, and social rehabilitation. The Clinical Quality Nurse supports the agency’s Continuous Quality Improvement Program and standards of care determined by regulatory bodies including the Joint Commission, HRSA, DPH, and others. This role supports change implementation across the agency by collaborating with clinical teams, supporting training needs, and Epic Optimization initiatives. With a focus on care improvement, the Clinical Quality Nurse participates in efforts to achieve established performance metrics, develops and analyzes effective reports, and promotes data-driven decision-making to enhance clinical outcomes.

Requirements

  • Bachelor’s degree in nursing
  • an active, unrestricted Registered Nurse License in CT.
  • Minimum of 3 years of experience.

Responsibilities

  • Works collaboratively with a multidisciplinary team of quality and compliance, clinic staff and leadership, and IT as applicable, to develop and enhance patient care initiatives through quality improvement projects, using a full PDSA cycle.
  • Supports patient care improvement initiatives including Complex Care Management, closing care gaps, and interagency program workflows.
  • Responsible for UDS data collation and evaluation, for improved patient care outcomes each year.
  • Works collaboratively with IT and Epic to ensure accuracy and meaningful use of patient outcome data reports to support best patient experience and solid clinical decision making.
  • Acts as Epic specialist and trainer, supporting staff during onboarding and Epic Enhancement Packet implementation.
  • Supports Training Jira system management.
  • Uses the nursing process and evidence-based practice knowledge to evaluate systems, recommends improvement initiatives, and support implementation change as needed.
  • Supports nurse and support staff onboarding, training, and competency evaluation as required.
  • Oversee medical training across the organization, including CPR and First Aid, Narcan Administration, medical crisis intervention, medication safety, and other trainings as needed.
  • Leads medication management initiatives, including monitoring medication reconciliation efforts and EMR support for medical staff.
  • Leads medication related Incident Report investigations and staff training as needed.
  • Knowledgeable about current documentation standards related to patient safety and risk management, supports all departments in implementation of uniform documentation practices.
  • Responsible for the agency’s infection prevention program, including implementation of best practices for infection prevention, ongoing surveillance activities to identify potential sources of infection in the healthcare setting, and collaborating with public health authorities and other organizations to coordinate infection control efforts.
