About The Position

The Clinical Quality Lead will provide leadership related to quality of clinical services at Esperanza Health Centers. Sets goals and objectives as related to Esperanza’s strategic plan. Leads the clinical team and partners with population health team members to support and enhance Esperanza’s clinical care model to achieve the population health aim of the Quadruple Aim. Provides primary medical care services in the ambulatory and/or inpatient settings. The Clinical Quality Lead reports to the Chief Medical Officer and collaborates across sites and teams at Esperanza, especially with the Quality, Population Health, and Compliance and Risk Management, to advance the delivery of high quality primary medical care.

Requirements

Board certification in a medical specialty.
Two or more years of clinical primary care experience in a community health setting.
Current IL State License, Controlled Substance License, DEA Registration, CPR Certification.
Excellent verbal and written communication skills.
Excellent interpersonal, negotiation, and conflict resolution skills.
Excellent organizational skills and attention to detail.
Excellent time management skills with a proven ability to meet deadlines.
Strong analytical and problem-solving skills.
Ability to prioritize tasks and to delegate them when appropriate.

Nice To Haves

Background in quality or safety methodologies such as Institute for Healthcare Improvement, Lean, or Six Sigma preferred
Experience leading interdisciplinary teams to achieve population health metrics preferred
Bilingual English/Spanish preferred

Responsibilities

Participates in ongoing quality improvement activities and collaborates with Director of Quality, Chief Medical Officer, and site leadership in pursuing excellence in care, including as related to UDS and other standard metrics.
Assists and advises site leadership in execution and implementation of strategies.
Develops and maintains clinical guidelines based on professional society recommendations and most recent evidence base.
Ensures adherence to guidelines by providers of all disciplines.
Provides clinical consultation and supports the Quality Director for EHC’s clinical peer review process.
Provides clinical consultation for incident review/safety concerns if requested by Chief Medical Officer or Site Medical Director.
Works with Director of Risk Management and Compliance to perform root cause analyses and update EHC’s policies, procedures, and guidelines in accordance with HRSA and FTCA.
Assists Chief Medical Officer with recommendations on approval of clinical privileges for core skills in their specialty.
May be designated to approve privileges as needed.
Coordinate Esperanza trainings and continuing education opportunities within specialty.
Provides back-up answering service call coverage for patients as needed.
Manages ambulatory primary care medical practice by eliciting patient histories, performing physical examinations, providing accurate clinical diagnoses, prescribing evidence-based and cost-effective treatments and providing patient education in a team-based model.
Makes appropriate diagnoses and prescribes appropriate prescriptions and referrals consistent with Esperanza Health Center's medical practice guidelines.
Adheres to Esperanza Health Center's productivity standards, maintains and promotes patient satisfaction through acknowledgement that environmental, social, and cultural factors that influence one's health.
Prepares, implements and documents age specific preventative health schedules for patients.
Builds and maintains a medical practice with follow-up of return appointments and referrals, and other methods that assure continuity of care.
Maintains clinical competency through regular Continuing Medical Education and provides documentation of such to Esperanza Health Center's Administration upon request.
Takes call for Esperanza Health Center patients as assigned by the Medical Director.
Develops and maintains relationships with partner organizations and community leaders.
Builds the organization’s reputation through participating in community health education and public health promoting activities.
Collaborates with External Affairs department to assist with marketing, publicity, media relations, website content, and support members of grants team as requested.
Performs other duties as assigned.
