Under the general supervision of the Director of Quality, the Quality LPN position requires interaction/collaboration with individuals in numerous health care disciplines. The Quality Improvement Nurse is responsible for analyzing acute care quality programs, clinical performance metrics, and patient outcomes to identify opportunities for improvement. This role partners closely with physicians, advanced practice providers, nursing leaders, clinical staff, and operational teams to translate quality data into actionable improvement initiatives. The Quality Improvement Nurse serves as a clinical quality expert, using evidence-based practices, performance data, chart review, and interdisciplinary collaboration to improve patient safety, clinical outcomes, regulatory compliance, and the overall quality of acute care services.

Requirements

  • Graduate of an approved LPN school of nursing.
  • Current license as a LPN in the State of Louisiana.
  • Experience with quality improvement, patient safety, clinical outcomes, utilization, or performance measurement.
  • Strong analytical, organizational, and communication skills.
  • Clinical experience in an acute care setting and/or physician practices.
  • Experience working with hospital quality programs and performance metrics.
  • Experience collaborating directly with physicians and advanced practice providers.
  • Ability to interpret clinical data and translate findings into actionable improvement strategies.
  • Experience with electronic health records, quality reporting systems, and clinical data analytics.
  • Familiarity with CMS, LDH, AHRQ, and other applicable quality and regulatory frameworks.

Responsibilities

  • Analyze acute care quality metrics, dashboards, trends, and performance reports to identify opportunities for improvement.
  • Monitor key quality and patient safety indicators, including hospital-acquired conditions, readmissions, mortality, length of stay, clinical complications, and other applicable measures.
  • Conduct clinical record reviews and data validation to identify gaps in care, documentation, and adherence to evidence-based practices.
  • Identify trends, variations, and potential root causes contributing to unfavorable quality outcomes.
  • Prepare clear reports and presentations for providers, nursing leadership, quality committees, and executive leadership.
  • Assist with development and maintenance of quality dashboards and performance-monitoring tools.
  • Partner with physicians, advanced practice providers, nurses, and interdisciplinary teams to review quality performance and identify improvement opportunities.
  • Provide providers with meaningful, actionable feedback related to quality metrics and clinical outcomes.
  • Facilitate discussions regarding variations in care, evidence-based practices, and opportunities to improve patient outcomes.
  • Build collaborative relationships with clinical teams to promote accountability and continuous quality improvement.
  • Support providers and staff in understanding quality measures, benchmarks, and performance expectations.
  • Develop, implement, and evaluate quality improvement projects focused on acute care outcomes.
  • Participate in root cause analyses, failure mode analyses, case reviews, and other structured improvement methodologies.
  • Help develop action plans based on identified quality gaps and monitor progress toward improvement goals.
  • Evaluate the effectiveness of interventions using measurable outcomes and performance data.
  • Support evidence-based practice initiatives and standardization of clinical processes.
  • Identify opportunities to reduce variation and improve reliability of care delivery.
  • Monitor performance related to applicable regulatory, accreditation, and quality requirements.
  • Support preparation for regulatory and accreditation surveys and reviews.
  • Assist with ensuring compliance with organizational policies, clinical standards, and applicable quality requirements.
  • Maintain knowledge of current acute care quality measures, evidence-based practices, and regulatory expectations.
  • Educate providers and clinical staff regarding quality measures, patient safety initiatives, clinical standards, and performance trends.
  • Develop educational materials, presentations, and reports to communicate quality findings.
  • Present quality data and improvement recommendations to interdisciplinary committees and leadership.
  • Promote a culture of continuous improvement, patient safety, and data-driven decision-making.
