About The Position

The Clinical Programs Coordinator I serves as the Streetlight Program Coordinator and assists the Program Director with administering the program. This includes volunteer supervision, maintenance of the Streetlight REDcap database, and providing patient care for adolescent and young adult patients in both inpatient and outpatient settings. The incumbent assists the Program Director with the facilitation of volunteer selection, training, and daily briefing meetings. The incumbent maintains the program's social media, helps develop promotional materials, and assists with the management of inventory related to patient care.

Requirements

  • Bachelor's degree in an appropriate field; or an equivalent combination of education and experience.

Nice To Haves

  • Bachelor’s Degree from an accredited institution and at least two years of professional experience with youth, patient care with recommended experience with hospice, palliative care, and/or end-of-life support.
  • Continuing education either through a master’s program in a health care/palliative care related field.
  • Communication and organization skills.

Responsibilities

  • Assists Program Director with new volunteer orientation and provides ongoing education in palliative care, chronic and terminal illness, HIPAA compliance and best practices to provide psychosocial care to the target population.
  • Assists Program Director to provide leadership and oversight for volunteer project teams (CF, Oncology, Sickle Cell, Autoimmune, Transplant, and Gaming and Online Team).
  • Conducts briefing meetings (one hour each) with the volunteers that include a weekly education focus, announcements, psychosocial records, and facilitated patient care intervention discussion based on the progress of the patient.
  • Organizes and archives team education materials.
  • Coordinates volunteer graduation ceremony and other social events.
  • Assists the Program Director to provide bereavement support to volunteers.
  • Assists Program Director to coordinate external supportive services for volunteers in maintaining selfcare.
  • Creates volunteer organizational tools including record keeping databases, polls, surveys, and sign ups.
  • Provides psychosocial support to adolescent and young adult patients via inpatient and outpatient visits, personalized projects, Teen Lounge, special events, and communication and programming through the Streetlight Discord.
  • Provides end-of-life support including creation of legacy projects.
  • Provides support to Streetlight Project Teams (CF, Oncology, Transplant, and Sickle Cell, Autoimmune and Gaming and Online Team).
  • Records volunteer attendance.
  • Assists the Program Director in recruiting, screening, interviewing and orientation of new team members.
  • Assists the Program Director in reviewing EPIC charts for Streetlight patient.
  • Assists the Program Director in recording daily patient referrals from medical staff and consolidating information into task-lists for volunteers.
  • Assists the Program Director in managing Streetlight encrypted SharePoint site featuring weekly team educational readings, announcements, and educational web links.
  • Assists the Program Director in managing Streetlight Volunteer and Patient REDCap Database.
  • Assists the Program Director in maintaining inventory of program supplies.
  • Assists the Program Director in facilitating communication and collaboration with interdisciplinary team members to best meet the needs of patients enrolled in Streetlight.
  • Creates and designs social media content for Streetlight events and outreach.
  • Assists with creation of promotional materials of the Streetlight program including brochures, pamphlets, and other digital and printed materials.
  • Manages the Streetlight website, Instagram, and Facebook page, ensuring all quality and privacy standards are met.
  • Initiates and facilitates community relations to support Streetlight activities (e.g., in-kind gifts, food provided for events, printing services and discounts, etc.).
  • Assists with program research projects as needed.
