About The Position

The Clinical Program Coordinator – Women’s Midlife Health supports the delivery and growth of clinical and supportive services focused on women in midlife, including menopause care. Serves as a key access point for patients by coordinating Hoag provider schedules, facilitating specialty referrals within the Hoag network, and connecting patients to support programs, education, and Hoag’s community-based events. Ensures a seamless, supportive patient experience while managing administrative, scheduling, and program coordination activities across the service line, including maintaining accurate documentation, scheduling records, and referral workflows within the electronic health record. Responds to patient inquiries coming in from the Hoag website, phone lines and internal departments, serving as a primary point of contact to answer questions, guide patients through care pathways, and support their connection to the appropriate resources and services. Assists with the planning, implementation, and evaluation of programs and events designed to meet the unique health needs of women in midlife. Coordinates and supports group visits, workshops, education sessions, and community outreach events related to midlife women’s health. Collaborates closely with clinical teams, marketing, and community partners to promote services and programs, maintain website and digital content, and support participation in health fairs and other educational activities. Tracks program participation, patient satisfaction, and referral outcomes, compiles data and reports, and assists with program evaluation to identify opportunities for continuous improvement and enhance patient engagement and experience. Facilitates referrals to specialty services such as pelvic health, behavioral health, integrative medicine, nutrition, bone health, and cardiology, and connects patients to Hoag programs, education, and community resources as needed to support coordinated care across the service line. Performs other duties as assigned.
