About The Position

The medication history technician is a clinical pharmacy technician responsible for ensuring accurate and up to date home medication lists for admitted patients at WVU Hospitals. In this role, the technician collects medication information from patients, caregivers, pharmacies, and facilities and updates this information in the electronic medical record. It is critical that anyone assuming this role has a strong baseline knowledge of the top 200 medications, their brand/generic names, and common indications for use. This technician works closely with the inpatient clinical pharmacists. The position is typically Monday - Friday with potential for weekend staffing. Candidates must be available to work on-site in Morgantown with the opportunity to work from home after sufficient training. No holiday staffing is required.

Requirements

  • Must possess current licensure as required by state board where services will be provided: MD: Pharmacy Technician License or Pharmacy Technician Student Permit through Maryland Board of Pharmacy, OH: Certified or Registered Pharmacy Technician License or Trainee License through Ohio Board of Pharmacy, PA: Pharmacy Technician or Pharmacy Technician Trainee License through the Pennsylvania State Board of Pharmacy, WV: Pharmacy Technician License or Trainee License through West Virginia Board of Pharmacy. Remote Workers Only: Must additionally hold Pharmacy Technician License or Pharmacy Technician Trainee License issued by state of residence.
  • Nationally Certified Pharmacy Technician through Pharmacy Technician Certification Board (PTCB) or National Healthcareer Association (NHA)
  • Five (5) years of Pharmacy Technician or healthcare related experience
  • Must be able to use computers and software applications.
  • Must be able to work accurately, efficiently, and safely in a fast-paced environment.
  • Must be polite and respectful when communicating with staff, physicians, patients and families. Must approach interpersonal relations in a positive manner.
  • Must demonstrate flexibility to meet the needs of department or hospital in regard to changes in work volume, planned change, scheduling changes and emergency call-backs (i.e. weather or disaster).
  • Must be able to communicate effectively verbally and in writing and place high emphasis on customer service.
  • Strong baseline knowledge of the top 200 medications, their brand/generic names, and common indications for use.
  • Two or more years of retail experience or pharmacy technician training program preferred OR three years of inpatient pharmacy experience
  • Nationally certified pharmacy technician through PCTB within 12 months of hire

Nice To Haves

  • Associates degree OR Post high school education in a science field of study OR graduate of a pharmacy technician training program.
  • Knowledge of drugs, drugs dosing, side effects, indications, contraindications and proper administration preferred.
  • Knowledge of rules, policies and regulations related to the disbursal of pharmaceutical drugs preferred.

Responsibilities

  • Securing patient data based upon comprehensive data elements, financial education/counseling, insurance verification, provision of clinical information for pre-certification, authorization and third party claims management.
  • Interviewing patients for clinical, financial, and history information and arranging appropriate clinical care.
  • Working independently gathering and submitting patient information with oversight of a pharmacist as needed.
  • Performing pharmacy related functions in compliance with departmental policies and procedures that provide optimal pharmaceutical care.
  • Drafting various correspondence letters and sending to referral source when necessary.
  • Gathering and documenting patient demographics, insurance information, needs assessment, patient history, and other data as needed.
  • Initiating refill/follow up calls and scheduling delivery as needed.
  • Providing information and answers to patient/caregiver questions and referring to clinician when appropriate or requested.
  • Identifying all patients requiring pre-certification or pre-authorization at the time services are requested or when notified by another hospital or clinic department.
  • Contacting insurance company or employer to determine eligibility and benefits for requested services.
  • Maintaining patient records in a timely and complete manner.
  • Assisting the pharmacist with the process of filling and labeling the prescriptions.
  • Calculating initial measurements for compounds and maintaining compounds logs.
  • Providing monographs and other required medication guides with each prescription.
  • Maintaining proper storage of medications (i.e. security, temperature controlled items, etc.).
  • Demonstrating the knowledge and skills necessary to communicate to the infant, pediatric, adolescent, adult and/or geriatric patient per the established age-specific education/standard.
  • Working with coordinators and manager to develop and attain quarterly objectives.
  • Maintaining confidentiality according to policy when interacting with patients, physicians, families, co-workers and the public regarding demographic/clinical/financial information.
  • Communicating problems hindering workflow to management in a timely manner.
  • Initiating charge anticipation calculations. Accurately identifying anticipated charges to assure identification of anticipated self-pay portions.
  • Participating in educational programs to meet mandatory requirements and identified needs with regard to job and personal growth.
  • Attending departmental meetings and/or documenting review of meeting minutes.
  • Participating in quality activities as assigned by Pharmacist Manager.
  • Participating in performance improvement (i.e. follows established work systems, identifies deviations or deficiencies in standards/systems/processes and communicates problems to coordinator or manager).
