About The Position

Responsible to develop, institute, and maintain patient-oriented programs, education, and professional staff development which promotes safe and efficacious drug usage across Bronson Healthcare Group. Develops, promotes, coordinates and oversees the clinical activities of the Pharmacists at all sites of clinical practice. Conducts P&T/PNT Committee meetings in order to maintain Bronson drug formularies to ensure safe, effective, and cost-efficient therapies are available for patient care. Employees providing direct patient care must demonstrate competencies specific to the population served.

Requirements

  • Bachelor's degree in Pharmacy or PharmD with 3 years of advanced clinical experience required
  • Licensed Pharmacist in good standing with the state of Michigan or license eligible.
  • Basic word processing and spreadsheet skills able to operate hospital programs used in the Pharmacy
  • Must be able to communicate well verbally and in writing

  • Residency training preferred
  • Track record of residency preceptorship and pharmacy program faculty appointment preferred

Responsibilities

  • Identifies areas of pharmaceutical use where usage evaluation and intervention are warranted based on high use, high risk, high cost or a combination of these factors.
  • Participates in development and implementation of pharmacy programs across the Bronson system to evaluate and control pharmaceutical use
  • Works closely with medical staff members in the clinical evaluation of patients and resolution of drug therapy problems. Makes recommendations on drug therapies and conducts follow-up. Monitors patients on drugs with narro therapeutic indexes.
  • Coordinates and conducts pharmacokinetic dosing services; provides instruction regarding appropriate application of kinetic principles to staff clinical pharmacists when using the software approved by the Bronson Pharmacy.
  • Participates in the development and use of appropriate monitors for Bronson Pharmacy Clinical Services. Assists in route cause analysis when indicated for adverse drug reactions (ADR) characterized as avoidable.
  • Provides instruction regarding a structured approach to Drug Information and Education to Residents and Staff Pharmacists.
  • Responsible for ensuring the decentralized pharmacists (DCP) receive adequate support and education to perform their jobs in an effective, rewarding manner and new pharmacists are oriented to the Drug Use Monitors (DUM) and Target programs. Assists/trains DCP's to detect and document adverse drug reactions.
  • Provides clinically oriented staff development programs that are relevant to the practice of the decentralized pharmacists at Bronson Health Group
  • Works effectively with pharmacy students assigned to Bronson to meet the educational goals established and directs the students to projects of interest to the staff at Bronson.
  • Provides effective leadership regarding Clinical Pharmacy activities to Pharmacy and Therapeutics Committee, staff pharmacists, physicians, and other members of the healthcare team at all practice sites.
  • Coordinates activities of decentralized pharmacists to assure attainment of established goals for clinical services. Directs drug use monitoring/Target programs of DCP's.
  • Serves as a pharmacist for Bronson at Home Infusion Services. This includes day to day home infusion pharmacy operations, drug order verification, prospective and retrospective drug utilization reviews, drug distribution, drug procurement, inventory management, patient counseling and coordination of follow-up related to ongoing therapy and labs
