The Medication Care Coordination Infusion (MCC) Pharmacy Team is a centralized team currently supporting 28 Infusion Departments across Corewell Health West (CHW), Corewell Health South (CHS) and Corewell Health East (CHE). Tasks within these areas of work include serving as the first-line resource for infusion pharmacy, scheduling, nursing, and provider teams for patient insurance benefit investigation, infusion pre-authorizations, navigating site of care restrictions, externally supplied medications, and insurance formulary preferences while troubleshooting infusion claim payment denials. The role coordinates educational programs and activities designed to enhance clinical drug knowledge of staff pharmacists, physicians, nurses, and other care providers. It works with staff pharmacists to increase their participation in drug monitoring, pharmacokinetics, and cost containment activities. The specialist provides pharmaceutical care to patients in assigned areas, implements cost containment programs, provides drug information, education, and clinical services that enhance the effectiveness of drug therapy and minimize patient risk. The role also works with multiple medication automation systems, evaluates patients and orders, and takes a lead role in the ordering and interpretation of drug serum levels using pharmacokinetic principles. The specialist provides physicians with dosage regimen recommendations to enhance therapeutic outcomes, reduce patient risk, and reduce the cost of drug therapy. Additionally, the role provides drug information and educational services to hospital health care professionals, patients, and families, and assists staff pharmacists in responding to drug information requests. It serves as an information resource and clinical practice role model for staff pharmacists. The specialist coordinates and implements departmental drug utilization review efforts and other process improvement activities as assigned, ensuring that processes and services are continuously monitored for quality, cost-effectiveness, and efficiency. The role engages in process and quality improvement activities, makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility. It facilitates applicable Pharmacy Committee activities in the area of specialty and participates on other committees/task forces as a member, consultant, and/or chair as assigned. The specialist engages in continuous study of the entire professional field to maintain professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. This includes completing required continuous training and education, including department and Corewell Health specific requirements on a timely basis. The role coordinates the development, editing, and dissemination of departmental publications for physicians and hospital staff, and precepts pharmacy residents and students as assigned.
