Requirements

  • Bachelors Pharmacy or Doctorate Pharmacy
  • RPH - Reg Pharmacist
  • Licensed by, and in good standing with, the Georgia State Board of Pharmacy or ability to obtain a Georgia State Board of Pharmacy License within 6 months of hire date within 180 Days Required
  • Minimum 3 years years clinical experience or Completion of a PGY-1 Residency Required

Nice To Haves

  • Board Certification (e.g., BCPS, BCGP, etc.)
  • Specialty credentialing (e.g., Certified Pain Educator)
  • Minimum 2 years years of clinical pharmacy experience in Pain Management/ Palliative Care or Completion of a PGY2 Pain Management/Palliative Care Residency
  • Experience with substance use disorder, geriatrics, and/or oncology
  • Experience with nutrition support, anticoagulation management, and pharmacokinetics

Responsibilities

  • Providing recommendations and/or assuming responsibility for pain management related activities or other medication regimens as prescribed by physician.
  • Document clinical interventions on an ongoing basis.
  • Demonstrate a basic understanding of distributive responsibilities including computer functions.
  • Provide recommendations and/or manage pain medications or other medication regimens as prescribed by the provider.
  • Round with interdisciplinary teams (where applicable).
  • Provide on-call clinical pharmacy services as needed.
  • Perform medication management activities as outlined per Wellstar policies.
  • Provide drug information when needed or requested.
  • Develop and provide educational programs/inservices for health care practitioners and patients as required.
  • Participate as a preceptor/instructor for students from accredited colleges/schools of pharmacy.
  • Serve as a preceptor/instructor for the Pharmacy Residency Program (where applicable).
  • Assist in developing drug protocols.
  • Participate as a member of the Wellstar Pharmacy and Therapeutics (P&T) Committee, as well as other System and facility-specific committees as assigned or as deemed necessary.
  • Identify target medications/therapies, develop criteria, and implement data collection for Medication Use Evaluations (MUE) and other pain management-related studies/initiatives. Tabulate and present completed MUE data to the P&T Committee.
  • Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges where applicable.
  • Assist in preparing for and maintaining compliance with regulatory agency (e.g., The Joint Commission) standards/requirements.
  • Monitor quality metrics related to pain management patients and develop improvement strategies as necessary.
  • Performs other duties as assigned.
  • Complies with all Wellstar Health System policies, standards of work, and code of conduct.
