About The Position

The Intra-abdominal transplant Clinical Pharmacy Manager is responsible for the clinical, operational, educational, and research oversight of pharmacy services within the solid organ transplant program. This role ensures the delivery of safe, effective, and evidence-based medication therapy management, leads a team of clinical pharmacists, and collaborates with multidisciplinary teams to optimize patient outcomes. The manager is responsible for operational efficiency, staff development, quality improvement initiatives, and adherence to regulatory standards.

Requirements

  • Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
  • Current and unrestricted pharmacist license/registration in NYS.
  • Prior leadership or supervisory experience preferred.
  • Experience in academic or teaching hospitals is desirable.
  • Experience in research/performance improvement is desirable.
  • Excellent analytical, organizational, communication, and teaching skills.
  • Strong commitment to patient safety, evidence-based practice, and quality improvement.

Nice To Haves

  • Completion of PGY2 Residency in transplant highly preferred.
  • Board certification in transplant (e.g., BCTXP) preferred.

Responsibilities

  • Oversee clinical pharmacy services for transplant patients (pre-, peri-, and post-transplant)
  • Ensure optimal use and monitoring of: Immunosuppressants (e.g., tacrolimus, cyclosporine, mTOR inhibitors, etc.) and Antimicrobial prophylaxis and treatment
  • Develop and maintain clinical protocols, pathways, and order sets for complex medication regimens and drug interactions
  • Participate in multidisciplinary transplant rounds
  • Provide patient care across inpatient settings and the outpatient transplant clinic, ensuring continuity of care throughout the transplant journey.
  • Provide direct clinical support for complex medication management for transplant patients.
  • Participate in daily rounds to optimize pharmacotherapy decisions.
  • Develop, implement, and monitor evidence-based protocols, order sets, and guidelines.
  • Oversee operational pharmacy functions, including medication inventory, cost management, and workflow optimization.
  • Cross cover transplant services for liver, heart, lung.
  • Educate healthcare providers, pharmacists, and trainees on transplant best practices.
  • Participate in transplant rounds, transplant quality improvement committee meetings.
  • Develop and deliver educational materials and in-services on transplant pharmacotherapy and disease management.
  • Serve as a preceptor for PGY1 and PGY2 pharmacy residents and APPE (Advanced Pharmacy Practice Experience) students, providing clinical instruction, mentorship, and evaluation during transplant rotations.
  • Ensure compliance with state and federal pharmacy regulations and institutional policies.
  • Maintain adherence to accreditation standards (e.g., Joint Commission, ASHP).
  • Monitor and report adverse drug events, medication errors, and other safety metrics.
  • Prepare and submit regulatory, operational, and performance reports as required.
  • Lead and support research initiatives related to transplant pharmacotherapy.
  • Design and conduct performance improvement projects aligned with hospital goals, regulatory requirements, and national standards.
  • Collaborate with transplant physicians on protocol development, retrospective reviews, and prospective studies.
  • Support pharmacy residents, students, and fellows in research projects, publications, and presentations related to transplant medicine.
  • Maintain awareness of emerging literature, guidelines, and best practices to inform program development and evidence-based interventions.
