About The Position

Under general direction, the Clinical Pharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include interviewing and examining patients for drug evaluation; checking on drug adherence and side-effects; discussing symptoms and effects of prescribed drugs with patients and maintaining a drug history on patients interviewed. Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients. Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc. Participates in the development and revision of medication use policies and guidelines. Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records. The 2454 Clinical Pharmacist also performs other related duties as assigned. Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. Some positions may require employees to work directly with individuals within locked jails and psychiatric facilities, the homeless population and/or individuals who have or are carriers of infectious/communicable diseases.

Requirements

  • Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy. (Please attach a copy of license); License status must be "Valid and Current" according to search.dca.ca.gov/
  • Completion of six (6) months of a pharmacy residency program; OR
  • Completion of six (6) months of a post-graduate pharmacy fellowship program; OR
  • Two (2) years of professional level experience as a Clinical Pharmacist (equivalent to class 2454 Clinical Pharmacist within the City and County of San Francisco); OR
  • Possession of a Board Certification in a pharmacy specialty area.

Nice To Haves

  • Completion of a PGY2 Pharmacy Residency program in emergency medicine, critical care, or toxicology; OR two years of full-time professional level experience as a clinical pharmacist in an Emergency Department (ED) setting.
  • Completion of a PGY2 Pharmacy Residency program in psychiatry; OR two years of full-time professional level experience as a clinical pharmacist in a psychiatric setting.
  • Bilingual fluency in a variety of languages depending on the department’s needs. Only those eligibles that pass the bilingual proficiency exam will be considered for bilingual positions.
  • The ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department.
  • Recent and verifiable experience: (1) integrating, supporting and maintaining an Electronic Health Record (EHR) or Pharmacy Information System (PIS); and (2) implementing, developing, and evaluating Computerized Prescriber Order Entry (CPOE), within the last three years.
  • Demonstrated experience with the acute care medicine patient population.
  • Clinical and/ or lived experience with diverse communities.
  • Excellent oral and written communication and interpersonal skills.
  • Completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship.
  • Fluency in Spanish and/or Cantonese is highly desired.
  • Mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities.
  • Meets requirements for prescribing as required by Standardize Procedure for Ambulatory Care Pharmacist within San Francisco Health Network.
  • Completion of a psychiatric pharmacy residency.
  • Clinical experience working in substance use disorder or mental health settings.
  • Board Certification as a Psychiatric Pharmacist.
  • Possession of a valid Advanced Practice Pharmacist license in California.
  • Completion of a geriatric pharmacy residency.
  • Certification in Geriatric Pharmacy.
