About The Position

The Clinical Pharmacist participates in clinical programs and initiatives 50–90% of the time on a consistent schedule. The role ensures safe, rational, and cost-effective medication therapy while maintaining compliance with applicable federal and state rules, regulations, and standards set by accrediting agencies and regulating bodies. The position is based at Harbour View Medical Center in Suffolk, Virginia. The role qualifies for a $5,000 sign-on bonus and may qualify for a $5,000 relocation bonus based on location.

Requirements

• Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD.
• Maintain a current pharmacist license in the state or states where practicing.
• Hold a valid state-issued ID.
• Have advanced knowledge and skills to perform pharmaceutical assessments and develop patient management plans.
• Demonstrate relationship-building, medication product preparation, and hazardous drug handling skills.
• Know pharmacy operating systems and automation; have computer skills, including MS Office, spreadsheets, PowerPoint, and drug databases.
• Have advanced medication knowledge, including adverse effects, interactions, proper dosing, monitoring parameters, and cost.
• Demonstrate regulatory compliance, ability to develop SMART goals and objectives, and ability to use the SBAR format.
• Demonstrate medication education, analytical skills including database management and data presentation and interpretation, teamwork, attention to detail, critical thinking, customer service, excellent verbal and written communication, coaching and mentoring, organization, and stress management.
• Exhibit proficient use of all computer hardware and software.

Nice To Haves

• Advanced pharmacy degree, Doctor of Pharmacy, Master's degree, or residency.
• ACLS Advanced Cardiovascular Life Support certification.
• Immunization certification.
• Pharmacy residency or three years of equivalent experience.
• General pharmacotherapy certification (BCPS) or another certification approved by the Board of Pharmacy Specialties.

Responsibilities

• Participate in clinical programs to improve patient care, optimize medication therapy, and promote health and disease prevention.
• Perform medication reviews and medication therapy management to support safe and appropriate medication use and achieve defined patient care outcomes.
• Maintain proficiency in order processing and support medication distribution and pharmacy operations to ensure accurate and timely medication availability.
• Support cost-containment efforts related to medication utilization and effective use of pharmacy resources.
• Collaborate and communicate with pharmacy team members, other healthcare professionals, and leaders to deliver pharmaceutical services and support departmental and organizational programs.
• Identify, document, and resolve potential and actual adverse medication events; evaluate and report event data for trending and prevention when applicable.
• Provide medication education to improve patient and associate understanding and compliance, consistent with department guidance.
• Educate pharmacy staff and serve as a preceptor for students and residents as assigned.
• Process medication orders and review them for appropriateness, safety, and efficacy; perform necessary clinical support duties.
• Perform other job-related duties as required by the supervisor, subject to reasonable accommodation.
