About The Position

The clinical pharmacist, under the supervision of the Director of Pharmacy and Pharmacy Clinical Coordinator, will practice the profession of pharmacy in this state with all the rights and privileges of a Registered Pharmacist and will adhere to the Code of Ethics of the American Pharmaceutical Association and the Alabama Board of State Pharmacy. The clinical pharmacist will be expected to participate in multidisciplinary patient rounds, evaluate and optimize patient medication therapy, monitor and recommend appropriate antimicrobial therapy, calculate and monitor TPN therapy, pharmacokinetic dosing, renal dose adjustments, and other clinical pharmacy activities as needed.

Requirements

  • Must have a Bachelor degree in Pharmacy or Doctor of Pharmacy from an accredited School of Pharmacy
  • Must be licensed as a Registered Pharmacist by the Alabama State Board of Pharmacy
  • Must maintain continuing education as required for current controlled substance permit
  • Must be certified or willing to obtain certification as a parenteral pharmacist
  • Ability to communicate effectively in English, both verbally and in writing
  • Ability to communicate effectively in an emergency situation
  • Ability to communicate effectively with physicians and other healthcare providers

Responsibilities

  • Participate in multidisciplinary patient rounds
  • Evaluate and optimize patient medication therapy
  • Monitor and recommend appropriate antimicrobial therapy
  • Calculate and monitor TPN therapy
  • Perform pharmacokinetic dosing
  • Perform renal dose adjustments
  • Perform other clinical pharmacy activities as needed
  • Manage all information obtained with strict confidentiality to safeguard patient information from use by unauthorized personnel
  • Follow safety guidelines for self, patients, visitor, and employees
  • Follow infection prevention policies
  • Independently handle communications in a manner which decreases conflict

  • Recommend appropriate antimicrobial therapy as per literature, cultural and sensitivity data
  • Creatinine Clearance Monitoring-Adjustments – recommends dosage adjustments for patients with renal dysfunctions
  • Monitor patient therapy for therapeutic duplications and drug interactions
  • Recommend dosage adjustments based on clinical information/trials or alternative therapy
  • Research and evaluate new drug therapies in relation to cost and effectiveness
  • Develop protocols for drug use as needed
  • Perform staff pharmacist duties when needed
  • Adhere to the standards of the Compliance Plan
  • Demonstrate responsibility and accountability for maintaining quality care and work performance by attendance at departmental meetings and other team meetings as assigned
  • Participate in team/department quality improvement activities
  • Accept accountability for own work and team outcomes when appropriate
  • Report promptly any or unusual situations

  • Perform all the necessary computer operations to ensure proper documentation of patient medication on the patient medication profile
  • Interpret physicians’ orders for completeness and accuracy
  • Follow up if there is an incomplete order
  • Document interventions with in EMR
  • Check all IV preparations and medication fills to insure correctness of product dispensed
  • Perform basic function of the IV room
  • Perform basic function of the Unit Dose Technicians
  • Perform, check, and replace medication to the crash carts and Acudose dispensing machines along with proper documentation required when called upon
