Requirements

  • Doctor of Pharmacy (PharmD)
  • Current WA State Pharmacist license
  • Completion of an ambulatory practice pharmacy residency program OR (2) years of clinical ambulatory experience OR (10) years of pharmacy practice experience

Responsibilities

  • Collaborate with providers to deliver medication reconciliation, pain management, and chronic disease support
  • Counsel patients on safe, effective medication use -- both in clinic and at community outreach locations
  • Monitor therapeutic progress and document care in the electronic health records (EHR) system
  • Provide education for medical residents, providers, and the clinical team
  • Support programs like diabetes care, tobacco cessation, and anticoagulation therapy
  • Research and recommend evidence-based formulary changes to enhance access and care
