Requirements

Doctor of Pharmacy (PharmD)
Current WA State Pharmacist license
Completion of an ambulatory practice pharmacy residency program OR (2) years of clinical ambulatory experience OR (10) years of pharmacy practice experience

Responsibilities

Collaborate with providers to deliver medication reconciliation, pain management, and chronic disease support
Counsel patients on safe, effective medication use -- both in clinic and at community outreach locations
Monitor therapeutic progress and document care in the electronic health records (EHR) system
Provide education for medical residents, providers, and the clinical team
Support programs like diabetes care, tobacco cessation, and anticoagulation therapy
Research and recommend evidence-based formulary changes to enhance access and care
