This is a Full-Time opportunity for a Clinical Pharmacist to join the team at Surrey Memorial Hospital. The position is available until approximately September 2027 or until the return of the incumbent. The role involves providing individualized pharmacotherapy, counselling patients about their medication therapy, and collaborating with physicians and other healthcare providers to develop, implement, and monitor therapeutic plans. The Clinical Pharmacist will also assist in developing drug therapy guidelines, preparing publications, acting as a department resource person, and participating in medication use evaluations, quality improvement initiatives, and clinical drug trials. Additionally, the role includes providing mentorship and supervision to pharmacy students, residents, and new staff, ensuring accurate prescription validation and dispensing, and resolving medication safety concerns. The Clinical Pharmacist also performs drug distribution duties, including processing prescriptions, reviewing prescribed medication therapy, validating prescriptions, identifying safety concerns, contacting relevant parties to resolve discrepancies, documenting modifications, and dispensing medications.
