The Clinical Pharmacist works closely with physicians, advanced practice providers, nursing staff, and patients to ensure the safe, effective, and appropriate use of medications. This position is responsible for medication management, patient education, clinical monitoring, medication access support, and documentation within the electronic medical record (EMR).

Requirements

  • Doctor of Pharmacy (PharmD) degree from an accredited college or school of pharmacy.
  • Current Texas Pharmacist license in good standing.
  • Strong knowledge of medication therapy management and clinical pharmacotherapy.
  • Excellent communication, organization, and interpersonal skills.
  • Ability to work effectively in a team-based healthcare environment.

Nice To Haves

  • Clinical pharmacy experience in oncology, specialty pharmacy, or physician practice setting preferred.
  • Experience with electronic medical records (EMR) preferred.

Responsibilities

  • Review, evaluate, and optimize medication therapy plans based on patient-specific needs and evidence-based guidelines.
  • Provide medication counseling and education to patients and caregivers regarding treatment plans, side effects, and adherence.
  • Collaborate with providers and clinical staff to improve patient outcomes and support comprehensive patient care.
  • Monitor patient response to therapy, including medication effectiveness, adverse reactions, and drug interactions.
  • Perform medication reconciliation and maintain accurate medication records.
  • Assist with specialty medication management, prior authorizations, appeals, and patient assistance programs as needed.
  • Document clinical interventions, medication management activities, and patient encounters in the EMR.
  • Support quality initiatives and regulatory requirements, including applicable clinical performance measures.
  • Serve as a medication resource for providers, staff, and patients.
  • Maintain compliance with all federal, state, and organizational pharmacy regulations and policies.
  • Participate in multidisciplinary care planning and patient management activities.
  • Perform other duties as assigned.
