About The Position

The Clinical Pharmacist is responsible for ensuring the safe and effective use of medications within the correctional healthcare setting. Duties include reviewing prescriptions and medication orders for accuracy, appropriateness, and completeness; monitoring patient drug therapy; identifying clinically significant drug interactions, adverse reactions, therapeutic duplications, and dosage concerns; and ensuring compliance with prescribed treatment plans. The Clinical Pharmacist collaborates closely with physicians, nurses, and other healthcare professionals to optimize patient outcomes. Additional responsibilities include recommending therapeutic alternatives, managing and expanding the medication formulary, participating in interdisciplinary healthcare initiatives, and providing medication-related education and presentations to executive leadership and clinical staff.

Requirements

  • Doctor of Pharmacy (PharmD) degree from a program accredited by the American Council for Pharmacy Education (ACPE)
  • Current, valid, and unrestricted Pharmacist license issued by the Arizona State Board of Pharmacy
  • Current Primary Source Verification (PSV) from the Arizona State Board of Pharmacy
  • Minimum of one (1) year of experience as a Clinical Pharmacist or Pharmacist
  • Updated Resume
  • Copy of PharmD Diploma and/or Official Transcripts
  • Current Arizona Pharmacist License
  • Current PSV from the Arizona State Board of Pharmacy
  • Current BLS/CPR Certification issued by the American Heart Association (AHA) through in-person training
  • Tuberculosis (TB) Test Documentation
  • Hepatitis B Vaccination Documentation
  • MMR (Measles, Mumps, Rubella) Vaccination Documentation
  • Varicella (Chickenpox) Immunity/Vaccination Documentation

  • Completion of a PGY1 Pharmacy Residency

Responsibilities
