About The Position

Boston Medical Center is searching for a Clinical Pharmacist to join our Hospital at Home service. The clinical pharmacist will be the onsite pharmacy representative for the Hospital at Home program. They will be remotely integrated into the care team to provide clinical pharmacy services to a diverse patient population while taking into account the nuances of delivering care to patients in their own home. The patient population will closely resemble those on an inpatient internal medicine unit; all patients will require true inpatient level of care, but will have stable disease states that lend to keeping them in their home. The pharmacist will communicate with the care team and the patient via teleconferencing and will perform all standard aspects of inpatient pharmacy care: medication history and patient-supplied medication validation, order verification, drug therapy optimization, therapeutic drug monitoring, patient counseling, etc. As the lead pharmacist for this program, the clinical pharmacist will help shape the way pharmacy provides care for this unique patient population. The Clinical Pharmacist 7/7 is responsible for the provision of direct patient care. Direct patient care activities include improving patient outcomes by optimizing pharmacotherapy, ensuring medication safety, and promoting effective transitions of care. The Clinical Pharmacist 7/7 will have opportunity to participate in committees and project work, provide education and training to other health care providers, and precept Advance Pharmacy Practice Experience students. This is a permanent night position, and includes weekends and holidays.

Requirements

  • Requires a Doctorate of Pharmacy or Bachelors of Science in Pharmacy with an advanced degree PGY1 pharmacy residency or at least three years of relevant experience.
  • Valid license to practice pharmacy in the Commonwealth of Massachusetts by the hire date.
  • MA pharmacist licensure must be complete and verified by the date of hire.
  • Strong clinical knowledge pertinent to the specific practice area.
  • Strong interpersonal skills when interacting with patients and colleagues.
  • Proficient with Microsoft Office including Word, Excel and Outlook, and web browsers.
  • Excellent attention to detail.
  • Effective verbal and written communication skills; must be proficient in the English.
