About The Position

Work Schedule : 20% FTE. Day/Evening Shifts, varying shifts available. 8 hour shifts. You will work at University Hospital in Madison, WI. Hours may vary based on the operational needs of the department. Pay : Pay range is based on a full-time (100%) position. We are seeking a per diem Clinical Pharmacist to: Practice in a setting that supports career growth in clinical pharmacy. Opportunity to precept UW-Madison School of Pharmacy students. Provide medication therapy management services as part of an interdisciplinary team, to patients in an inpatient setting, to assure that drug therapy is safe, effective, efficient, and cost-effective. Requirements

  • Bachelor's Degree in Pharmacy or a PharmD degree
  • 1 year of clinical patient care and orders management experience
  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days
  • Basic Life Support (BLS) within 90 Days
  • Advanced Cardio Life Support (ACLS) within 180 Days

Nice To Haves

  • Completion of an ASHP Accredited Residency training program
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.)
