About The Position

The Clinical pharmacist is responsible for ensuring the safe, appropriate, and cost-effective use of medications. He/she provides expertise on drug therapy initiation and monitoring to all customer groups served by MercyOne North Iowa Medical Center, working collaboratively with other health care professionals to devise the most appropriate drug treatment for patients. The role of the clinical pharmacist sometimes extends outside the hospital, sharing their specialized knowledge with other hospitals, clinics, and care settings. Performs clinical review of patient in area of responsibility. Prospectively identifies and prioritizes potential and /or actual medication-related problems and/or alternative therapies. Provides information to medical staff, nurses, or other health care personnel concerning all aspects of clinical pharmaceutical information pertaining to the patient’s care and well-being, keeping in mind the confidential nature of all such information and communication. Recommends medication therapy changes to interdisciplinary patient care team members as necessary. Monitors patient medication therapy for pharmaceutical care parameters and documents (as required) the appropriate findings, recommendations, interventions, and/or outcomes. Provides accurate dispensing and timely distribution of medications for inpatients and outpatients. Compounds and dispenses medications and other pharmaceutical supplies using standard procedures to fill medication orders generated by qualified prescribers. Screens new medication orders for appropriateness, medication-related problems, and/or alternative therapies. This includes follow-up on incomplete orders, non-formulary orders, etc. Designs and/or adjusts medication therapy by Pharmacy & Therapeutics Committee direction or at the request of the provider. Counsels patients on the effects, dosage, and route of administration of their drug treatments, particularly those who require complex drug therapy. Completes all expected pharmacy functions, duties, and responsibilities prior to leaving assigned shift/period and work area. This includes using time at work in a productive manner. Perform Antimicrobial Stewardship activities as requested by the Antimicrobial Stewardship Committee. Abides by USP standards for sterile and non-sterile compounding. Keeps accurate and legible records and documentation forms. Provides functional supervision to the pharmacy operation in area of responsibility. Performs duties in a cost-effective manner, striving to avoid wasting resources. This includes using time at work in a productive manner, providing/implementing suggestions, and documenting activities that conserve resources (financial and human). Participates in quality improvement activities; e.g., process improvement, DUE/DUR, development of protocols, etc. Maintains professional education/development and assists in the education of other team members. Maintains required department confidentiality and abides by HIPPA regulations. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments. Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development, and participates in medical center and/or departmental education programs. Communicates with and performs duties following the philosophy of MercyOne and the Sisters of Mercy reflecting the organization’s Mission, Values, and Guiding Behaviors. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment. Performs other responsibilities as requested by supervisor, manager or department head.

Requirements

  • Completion of the requirements for a Pharm.D.
  • Iowa Pharmacist licensure, either by examination or reciprocity.
  • Demonstrated competence in pharmacy for 2-3 years, focusing on the provision of pharmaceutical care.
