About The Position

Trustbridge Hospice, a member of Empath Health, is seeking an organized and flexible Clinical Pharmacist to join our team in West Palm Beach. This position is onsite. This position works full time with weekends, holidays and on call. There is a rotating schedule and shift times are: 11:00am - 7:30pm, 2:30pm- 11:00pm. Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

Requirements

• Must be a Registered Pharmacist in the State of Florida.

Nice To Haves

• Consultant Pharmacist license preferred.
• One (1) year experience in institutional pharmacy practice preferred.

Responsibilities

• Ensures the safe and effective use of medications by providing medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services and documents all activity.
• All concerns, issues, or questions are clarified with the individual prescriber before dispensing the medication and assessments and interventions are documented.
• Serves as a pharmacological resource, providing in services and education
• Oversees the work of the pharmacy technicians and pharmacist interns to assure compliance with policy and procedures and accuracy of work performed as required by Florida law
