We’re looking for a Clinical Pharmacist who wants their work to mean something—someone who brings care, accountability, and purpose to what they do every day. As a Clinical Pharmacist, you play a critical role in ensuring patients receive safe, effective, and timely specialty infusion therapies. Your expertise helps guide medication management, support clinical decision-making, and ensure every prescription is reviewed with accuracy and care. In this role, you’ll contribute to work that: Helps patients feel informed, supported, and confident throughout their treatment journey Ensures safe and compliant specialty pharmacy operations Supports clinicians, providers, and care teams through clinical expertise and collaboration Advances positive patient outcomes through thoughtful medication management and oversight. Requirements

  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) or Master of Science in Pharmacy preferred.
  • Current licensure as a Registered Pharmacist in the State of New York.
  • Minimum of two years of recent experience in an acute care setting or one year of recent home infusion therapy experience preferred.
  • Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
  • Ability to evaluate and monitor drug therapy relative to the total needs and clinical conditions of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
  • Ability to work effectively with interdisciplinary teams to provide safe and effective drug therapy.
  • Demonstrated ability to problem solve and supervise pharmacy technicians and support staff.
  • Strong organizational and time management skills.

Responsibilities
