About The Position

Onco360 Pharmacy is a unique oncology pharmacy model created to serve the needs of community, oncology and hematology physicians, patients, payers, and manufacturers. Our Onco360 Pharmacy in Buffalo, NY has an immediate need for a Clinical Pharmacist. This is a remote role available with Monday to Friday, daytime hours. The salary starts at $138k annually and up depending on experience, with bonus opportunities. The Clinical Pharmacist provides appropriate recommendations for drug therapy and contributes to the optimal medication management of patients. They review, interpret, verify, and dispense medications and other pharmaceutical supplies. This role also involves completing patient medication therapy management (MTM) calls for adherence and adverse event monitoring, providing clinical pharmacy services, and responding to medical information requests.

Requirements

  • Bachelor’s Degree in Pharmacy or Pharm D
  • One or more years of specialty pharmacy practice experience
  • Knowledge of pharmacy laws and federal and state regulations concerning long-term care and pharmaceutical care practices for long-term care residents.
  • Ability to work cooperatively as a member of a team.
  • Current pharmacist licensure in at least one state or US territory

Nice To Haves

  • Completion of PGY-1 and/or PGY-2 pharmacy residency training in oncology
  • One or more years of oncology pharmacy practice experience
  • Credentialing from Board of Pharmacy Specialties (BPS) such as Board Certified Oncology Pharmacist (BCOP) or Board Certified Pharmacotherapy Specialist (BCPS), credentialing from National Association of Specialty Pharmacies as Certified Specialty Pharmacist (CSP)

Responsibilities

  • Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate.
  • Serves as lead resource to internal stakeholders for medical/drug information inquiries.
  • Maintains patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).
  • Ensures that appropriate laboratory tests for monitoring certain high risk conditions are ordered, evaluated and discussed with appropriate members of the healthcare team.
  • Collaborates with the healthcare team to provide clinical and technical support as needed.
  • Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record.
  • Responds to patient-specific or non-patient-specific inquiries from internal or external stakeholders for drug/medical information related to any medication within company’s business portfolio.
  • Completes Medication Therapy Management (MTM) program calls with patients/caregivers taking certain oral, injectable, and/or intravenous oncology products for the purposes of adherence and adverse event monitoring. Provide appropriate recommendations within pharmacist scope of practice as to mitigate non-adherence or the occurrence of adverse events. Provide comprehensive education to patients/caregivers regarding a medication product, disease state, manufacturer-based services, etc.
  • Documentation of adverse events and products complaints per contractual obligations to pharmaceutical manufacturers.
  • Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct and Ethics, its policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.
  • Assists facility/agency staff personnel with procedures relating to safe and effective methods of procuring, maintaining, administering and disposing of drugs.
