About The Position

ChristianaCare is seeking a full-time Clinical Pharmacist to join their inpatient pharmacy team. This role focuses on providing direct patient care to high-acuity and step-down ICU patients. The position involves participation in daily interdisciplinary rounds, management of specific medication therapies, evaluation of drug efficacy and outcomes, and ordering/evaluating laboratory tests. The pharmacist will also communicate recommendations to providers, document interventions, and potentially serve as a preceptor for pharmacy students and residents. Responsibilities include providing drug information, assisting with projects like MUEs and drug monographs, reviewing provider orders, dispensing medications, and supervising workplace activities. The role also involves assuming responsibility for satellite pharmacies, including sterile product processing in the IV Admixture Service lab and OR pharmacy. This is a full-time position (40 hours weekly, 8-hour shifts) primarily working day shift, with an evening shift component and a requirement for weekend and holiday coverage focused on ICU care.

Requirements

  • Bachelor of Science degree in Pharmacy or PharmD degree from an accredited College of Pharmacy required.
  • Current licensure (or eligible to reciprocate) in the State of Delaware required.

Nice To Haves

  • ASHP-accredited pharmacy residency preferred or equivalent experience (3 years acute care).
  • Board certification preferred.

Responsibilities

  • Participate in direct patient care through daily interdisciplinary rounds.
  • Manage specific medication therapies per approved policies (e.g., warfarin, aminoglycosides, vancomycin, IV to PO conversions, renal dose adjustments, indication-directed dosing adjustments, selection of surgical prophylaxis medications).
  • Evaluate drug efficacy, monitor therapeutic goals and outcomes, and review for contraindications/precautions.
  • Order and evaluate laboratory tests per approved protocols to support medication therapy monitoring.
  • Identify opportunities during chart review for counseling and medication history completion.
  • Communicate recommendations directly with provider.
  • Electronically document interventions and provide sign out as appropriate.
  • Serve as a preceptor for both pharmacy students and residents.
  • Provide drug information to allied healthcare professionals and fulfill provider-initiated pharmacy consults.
  • Assist with projects such as MUEs, drug monographs, and performance improvement activities.
  • Review and interpret provider orders and dispense medication.
  • Coordinate and supervise work-place activities.
  • Assume responsibility of the satellite pharmacies including the sterile product processing in the IV Admixture Service lab and OR pharmacy.
