About The Position

A rural hospital located on the Western Slope of Colorado is seeking a Clinical Pharmacist for their Inpatient Pharmacy. This community-based hospital offers Cancer Care, multiple Cath labs, and most specialties, with a primary goal of keeping patients "at home" for their healthcare needs. The role involves preparing and dispensing medication orders, maintaining medication stock, and ensuring safe, appropriate, and cost-effective medication therapies.

Requirements

  • Current Colorado pharmacist license or eligibility to obtain
  • 2-5 years pharmacist experience IN A HOSPITAL SETTING
  • Experience with CODE BLUE response
  • Experience with medication reconciliation and drug therapy management
  • Willing to work night shift if required
  • Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree.
  • Current license to practice pharmacy in the state of Colorado
  • BLS (American Heart Association)
  • ACLS (American Heart Association)

Responsibilities

  • Interprets medication orders; accurately transcribes to, and maintains computerized patient medication profiles.
  • Accurately compounds and dispenses pharmaceuticals, including IV admixture preparations, chemotherapy, and parenteral nutrition products.
  • Complies with all applicable federal, state and local laws and regulations.
  • Issues controlled substances to patient care area and maintains records as required by law.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
  • Performs pharmokinetic drug dosing per hospital protocol.
  • Reviews/interprets C&S data for antibiotic appropriateness, recommending changes as needed.
  • Accurately reads, extracts, and interprets patient chart information.
  • Accurately detects and reports adverse drug reactions.
  • Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Provides clinical consultation and clarification to practitioners.
  • Suggests appropriate cost-effective therapeutic alternatives to medical staff, as needed.
  • Provides drug information to patients and families as needed.
  • Accurately documents all clinical activities and interventions.
  • Participates in the quality improvement and medication review activities of the department.
  • Collects data; conducts quality monitors and inspections; maintains logs, records and other documentation as assigned.
  • Participates in the development and presentation of orientation, education and training programs to the pharmacy, nursing, medical and other staffs, as needed.
  • Member of Code Response Team.
