About The Position

The Clinical Pharmacist provides comprehensive pharmaceutical care to patients by delivering clinically oriented pharmacy services. These responsibilities include patient education and counseling of medications, performing medication histories and ensuring optimum patient outcomes or therapy. In addition, provide training and teaching to pharmacy students and residents, medical students and residents, and ancillary professional staff, i.e., dieticians, nurses, respiratory therapists and others. This position is grant-funded until December 31, 2029.

Requirements

  • Doctor of Pharmacy Degree from a college of pharmacy accredited by the American Council on Pharmaceutical Education or equivalent
  • Licensed as a Pharmacist in the State of Illinois in good standing
  • Successfully completed a PGY1 (Clinical Pharmacy Residency)
  • Proficient in Microsoft Office (Word, Excel, PowerPoint, etc.) and clinical applications
  • Prior experience in both an outpatient and inpatient facility, hospital or center
  • Board Certified Pharmacotherapy Specialist (BCPS)

Nice To Haves

  • One (1) year of comparable clinical pharmacy experience
  • Ambulatory Care Pharmacy Specialty Certification (BCACP)
  • Board of Pharmacy Specialty (BSP) certification
  • Pharmacy Residency PGY2 or minimum of three (3) years of experience in both an outpatient and inpatient facility, hospital or center
  • Advanced Cardiovascular Life Support (ACLS) certification

Responsibilities

  • Monitors drug therapy by routinely analyzing patients' drug therapy with respect to potential problems, laboratory data and special procedures
  • Provides consultation on the selection of drug therapy and appropriate dosing regimens, determination of therapeutic endpoints, total parenteral nutrition, clinical pharmacokinetics and intravenous therapy
  • Participates in patient care rounds, as well as attending and grand rounds
  • Monitors and evaluates patients' drug therapy in the ambulatory setting of high-risk meds (for Clinical Pharmacists)
  • Prepares patient admission medication histories on selected patients using accepted guidelines and document this information in the patient’s permanent medical record using the approved format in accordance with the established policy and procedure
  • Provides patient education and counseling on selected patients using accepted guidelines
  • Participates in the education and training of pharmacy staff and pharmacy trainees (i.e., pharmacy students and residents)
  • Initiates and/or participates in pharmacy and clinical drug investigations
  • Conducts drug utilization evaluations and participate in patient continuous quality improvement projects
  • Serves as the department of pharmacy representative on various committees throughout the institution (i.e., Pharmacy and Therapeutics subcommittees, Task Force committees)
  • Prepares literature evaluations for new drugs being requested through the Pharmacy and Therapeutics committee
  • Contributes written information for Pharmacy Newsletter
  • Remains current with medication and pharmaceutical literature
  • Maintains state of Illinois continued education (CE) hours at an annual minimum of 15 C.E contact hours per year
  • Records and submits workload statistics/clinical interventions as required by departmental administration
  • Participates in the management of medical emergencies, i.e., disasters, cardiopulmonary resuscitation
  • Receives verbal medication, or medication related orders from authorized practitioners and reduce them to writing on a Physicians' Order Sheet (POS) or input into Cerner, in accordance with established policies and procedures
  • Monitors, detects and reports adverse drug reactions (ADR's) and medication administration errors in accordance with established policies and procedures
  • Assists in proper functioning of the automated drug distribution system. Including but not limited to monthly inspections of clinical areas
  • Audits patient care areas for outdated medications, proper handling of pharmaceuticals, including controlled substances and emergency supplies
  • Performs job functions of a staff pharmacist, when necessary
  • Maintains the work area in a clean and orderly manner
  • Reports to the Pharmacist Manager
  • Performs other duties as assigned
