About The Position

The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry-level position into the practice of clinical pharmacy in an alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff, and other professionals regarding appropriate medication use. The role involves determining patient suitability for home care, designing and monitoring patient-specific pharmacy care plans, assessing nutritional needs, providing medication-use education, ensuring continuity of pharmaceutical care, preparing and dispensing medications, managing medication administration equipment, responding to drug information requests, participating in continuing education, managing investigational drug products, and effectively utilizing pharmacy support personnel. The position also requires adherence to all legal, regulatory, and safety requirements, including maintaining patient confidentiality.

Requirements

  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
  • At least one year of experience in sterile IV admixture procedures OR knowledge of non-sterile compounding, immunizations and medication therapy management (MTM) OR completion of an ASHP accredited Pharmacy Residency Program OR completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy OR equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
  • Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction.
  • Basic level skill in Microsoft Word.
  • Basic level skill in Microsoft Excel.
  • Basic skill level in Microsoft PowerPoint.

Nice To Haves

  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Experience in home care, hospital or other clean room setting.
  • At least 1 year of experience in indirect management of team members, including assisting in the development, training and assignment of work/projects to other team members.

Responsibilities

  • Determines the suitability of individual patients for home care by collecting and organizing patient-specific information.
  • Assesses patients' suitability for home care based on medical condition, prescribed medication therapy, and prognosis with clearly defined outcome goals.
  • Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
  • Makes recommendations for modifications in prescribed medication therapy or monitoring parameters when appropriate.
  • Collects and organizes patient-specific information to prevent, detect, and resolve medication-related problems.
  • Completes nutritional assessments of patients and makes appropriate recommendations.
  • Modifies pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information.
  • Demonstrates competence in the use of pharmacokinetic principles and formulas, and in assessing patient laboratory values.
  • Provides medication-use education to Option Care staff, other professionals, home care patients, and/or their caregivers using effective patient education techniques.
  • Ensures continuity of pharmaceutical care to and from the home and other patient-care settings through systematic communication.
  • Prepares and dispenses medications using appropriate techniques, following Option Care policies and procedures, and adhering to safety and quality assurance practices.
  • Calibrates equipment accurately and prepares medications that are appropriately concentrated, without incompatibilities, stable, and appropriately stored.
  • Prepares labels that conform to organizational policies and procedures, ensuring all necessary ancillary labels are present.
  • Inspects the final medication before dispensing and assures authorization to dispense.
  • Complies with state/federal laws and USP standards, including DEA controlled substance regulations.
  • Complies with policies regarding perpetual inventory systems for controlled substances and other medications subject to diversion.
  • Manages the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment.
  • Accurately programs, maintains, and advises others on the use and maintenance of all medication administration and medication-related equipment.
  • Participates in the process for assessing, managing, and reporting medication-related incidents and equipment errors/failures.
  • Accurately completes QAR documents according to Option Care policy.
  • Monitors and reports discrepancies in perpetual inventory systems for controlled substances.
  • Provides concise, applicable, and timely responses to drug information requests.
  • Attends educational seminars or participates in other CEU opportunities to promote professional competency and comply with state-mandated CEU requirements.
  • Manages the use of investigational drug products according to established regulations and Option Care policies and procedures.
  • Utilizes pharmacy support personnel effectively by prioritizing workload, organizing workflow, and ensuring accuracy of their work.
  • Maintains confidentiality of patient and proprietary information, observing legal and ethical guidelines.
