About The Position

The Clinical Pharmacist – Med/Surg/Central provides care to patients across Providence Portland Medical Center, a 555-bed community teaching hospital and tertiary referral center in Portland, Oregon. The pharmacist collaborates with multidisciplinary healthcare teams to provide direct patient care and high-quality, individualized, cost-effective pharmaceutical care integrated with pharmacy operations. The role focuses on optimizing patients’ medication therapy outcomes and quality of life. There may be opportunities to precept students and residents for interested pharmacists. The position works 10-hour shifts across both days and evenings.

Requirements

• PharmD in Pharmacy or a Bachelor's Degree in Pharmacy.
• Oregon Pharmacist License upon hire.

Nice To Haves

• Acute care hospital pharmacy experience, one year or equivalent experience.
• Completion of an ASHP-accredited PGY1 residency.

Responsibilities

• Provide direct patient care across the hospital in collaboration with multidisciplinary teams.
• Provide high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services.
• Optimize patients’ medication therapy outcomes and quality of life.
• Precept students and residents if interested.
