About The Position

The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient. Requirements

  • Doctorate Doctor of Pharmacy or RPH
  • Knowledge of pharmacokinetic principles required for medication dosing
  • RP - Registered Pharmacist - State of Pennsylvania or RP - Registered Pharmacist - State of New Jersey
  • Certification required based on primary work location.

  • 1 year PGY1 Pharmacy Residency preferred
  • Microsoft Word and Microsoft Outlook skills

Responsibilities

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Assures the appropriateness of drug therapy and compliance with hospital policies
  • Oversees the distribution of medications throughout TJUH
  • Supports implementation of processes to assure the quality of Pharmacy services
  • Trains staff as assigned (e.g., residents, interns, technicians, new pharmacists)
  • Precepts student pharmacists as assigned. See Preceptor Development & Evaluation Program policy.
  • Monitor patients for response to medications
  • Identify and document sentinel/serious events, near misses, medication errors and adverse drug reactions in the electronic reporting system
  • Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee.
  • Provide care that is appropriate to the chronological age, developmental age, social maturity, and cultural background of the patient
