Requirements

  • Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
  • 2 years of relevant experience hospital pharmacy

Responsibilities

  • Maintains accountability for ensuring the safety and efficacy of medication use.
  • Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy.
  • Participates in process improvement activities and drug use evaluations.
  • Works with multiple medication automation systems.
  • Provides comprehensive clinical pharmacy services across ICU, progressive care, and emergency department settings, including participation in codes, rapid response events, and acute stroke response.
  • Ensures safe and effective medication use through pharmacokinetic dosing, antimicrobial stewardship collaboration, and medication safety oversight within assigned patient care areas.
  • Collaborates routinely with interdisciplinary team members to support optimal patient outcomes.
  • Serves as a preceptor and mentor for PGY-1 and PGY-2 pharmacy residents, promoting clinical development and professional growth.
  • Participates in residency program activities, including medication use evaluations (MUEs), research initiatives, and acute care skills training for resident onboarding.
  • Contributes to PGY-2 critical care residency programming through precepting, MUEs, and research mentorship.
  • Performs inpatient pharmacy operational responsibilities.
  • Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
  • Safely compounds and dispenses medications for administration to patients.
  • Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
  • Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
  • Engages in continuous study of the entire professional field to maintain professional competence, knowledge, and skills.
  • Completes required continuous training and education.
  • Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
  • Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
