Responsible for the optimization of drug therapy by identification, resolution, and prevention of drug related problems and improving therapeutic outcomes. Also responsible for providing patient care according to the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. This role has principal antineoplastic stewardship duties and responsibilities, including developing and enforcing policies, coordinating programs to minimize medication errors, supporting the stewardship program through audits and evaluations, serving as a pharmacy consultant, and representing Pharmacy Services on various committees. Additional responsibilities may include prescribing medications under collaborative drug therapy management agreements, supervising support personnel, ensuring compliance with drug programs, documenting interventions, and participating in quality improvement efforts.
