About The Position

The primary service will be to support the filling of prescriptions for patients being discharged from the PWMC campus. The pharmacist serves as part of the healthcare team by supporting authentic personalized relationships with patients and their chosen support system, collaborating with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health. The pharmacist accepts responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures. As a member of the UVA Community Health team, individuals contribute to patient care decisions, support advanced medical technologies, and experience the satisfaction of making a difference in people’s lives every day.

Requirements

  • 4 Year/Bachelor's Degree required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification.
  • 1 year of experience required - Postgraduate year 1 residency or fellowship OR a minimum of 2 years of experience as a registered pharmacist in health system pharmacy.
  • Current Pharmacist licensure in the state of Virginia completed within 30 days of start date by either examination or reciprocity.
  • Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years of start date as applicable to the role required; excludes PRN team members who may not be eligible to take certification exam.
  • American Heart Association (AHA) Health Care Provider BLS certification required.
  • American Heart Association (AHA) Health Care Provider ACLS certification - within six months of hire required.
  • Ability to drive/travel to multiple facilities/locations as needed.
  • Ability to work as part of a high performing, change ready and resilient team.

Responsibilities

  • Support the filling of prescriptions for patients being discharged from the PWMC campus.
  • Collaborate with team members to assess, plan, implement and evaluate an individualized plan of care that promotes optimal health.
  • Accept responsibility, authority, accountability and ownership for management and provision of care in accordance with current policies and procedures.
  • Provide optimal medication management and promotion of medication safety.
  • Perform patient assessments.
  • Participate in direct patient care activities and rounding opportunities where available.
  • Monitor therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions.
  • Review and act upon clinical decision support surveillance.
  • Complete organizational and/or departmental required documentation.
  • Prescribe and/or administer medications as allowed by law and scope of practice.
  • Maintain up-to-date, evidence-based knowledge required for services provided.
  • Ensure timely and consistent communication.
  • Ensure timely responses to service line requests and inquiries.
  • Participate in patient education and wellness-based initiatives as driven by scope of practice.
  • Check and control quality and accuracy of work performed by pharmacy team members.
  • Prioritize workflow ensuring all services are performed in a timely manner.
  • Maintain and follow formulary approvals, interchanges, stocked medications, and non-formulary process.
  • Monitor and enhance compliance with clinical programs.
  • Develop and deliver effective education to pharmacists and other healthcare providers.
  • Serve as a preceptor to residents and students.
  • Participate in the onboarding of pharmacists and other team members.
  • Provide ongoing feedback to fellow pharmacists to achieve desired outcomes.
  • Serve as a mentor to pharmacy team.
  • Perform additional duties as assigned.
