As a Clinical Pharmacist, you will play a crucial role in ensuring the safe and effective use of medications for patients. This involves reviewing prescriptions for potential drug interactions, correct dosing, patient allergies, and any unusual findings, acting as the final expert check. The role requires a proactive approach to learning, real-time research, and a commitment to providing exceptional patient experiences.

Requirements

  • B.S. Pharmacy or Pharm D.
  • Valid Arizona pharmacist license
  • American Heart Association-issued Basic Life Support for Healthcare Providers (CT CPRBLS)

Nice To Haves

  • One year hospital experience
  • Strong clinical pharmacy skills
  • Previous experience with Hospital Pharmacy computer systems

Responsibilities

  • Document clinical activities and interventions such as consults, patient teachings, and other patient-specific information as defined by policy and procedure.
  • Interpret medication orders and/or prescriptions for drug interactions, appropriate drug selection, and appropriateness.
  • Accurately enter/verify the order or prescription into the computer system, label, and dispense medications in a timely manner.
  • Verify and check medications prepared by other pharmacy personnel.
  • Communicate drug information to patients and health professionals.
  • Resolve drug-related problems by consulting with other healthcare professionals to achieve appropriate patient outcomes.
  • Clarify unclear orders written by a licensed provider with the licensed provider.
  • Assist the licensed provider in appropriate drug selection and therapeutic regimens.
  • Achieve and maintain an adequate basic knowledge base to provide clinical services for all age groups served.
  • Provide basic clinical services as required by policy and procedure.
  • Maintain a working knowledge of automated systems.
