About The Position

Under the supervision of the Program All-inclusive Care for the Elderly (PACE) PACE Medical Director, the PACE Clinical Pharmacist is responsible for a wide range of medication management services and processes provided through the PACE Center. The Clinical Pharmacist must be able to make evidence-based recommendations for optimizing drug therapy for our participants. The PACE Clinical Pharmacist will be responsible for: PACE Enrollee Comprehensive Medication Review, Transitions of Care Medication Reconciliation, Anticoagulation Management, Medication Compliance & Safety, CMS Fraud/Waste/Abuse, Part-D quality and safety compliance activities, drug utilization reviews, PBM (Prescription Benefits Manager) reporting and review, Acumen safety reporting & compliance, and PACE Interdisciplinary team (IDT) representation and participation.

Requirements

  • A Doctoral degree in Pharmacy (PharmD) preferred or Bachelor of Pharmacy with BCPS Specialty
  • Must have a current, valid and unrestricted license to practice Pharmacy in New Jersey
  • NJ Pharmacist immunization License, must be able to obtain Collaborative License with NJ BOP
  • Strong clinical and operational pharmacy background; minimum of 2 years active pharmacy experience; long-term care pharmacy experience preferred; must demonstrate extensive knowledge of current regulations (USP, NJ Board of Pharmacy, NJ DOH, Joint Commission, CMS, DEA), and ASHP best practice.

Nice To Haves

  • Completion of a Pharmacy Practice Residency and/or BCPS Specialty in geriatrics, ambulatory care, or managed care.
  • Experience with a PACE Program or ambulatory care/primary care setting working with geriatric specific medication therapy management services

Responsibilities

  • Coordinates and oversees PACE Center and PACE Clinic medication management services, including comprehensive medication reviews, and medication reconciliations for PACE enrollees
  • In collaboration with the PACE Inter-Disciplinary Team, PACE providers and Atlanticare's MTDM pharmacists follows and updates medication regimens, utilization, safety/compliance during care transitions (hospital, ER, SNF, hospice): conducts Transitions of Care medication reconciliations and follows up on discharge medication as appropriate
  • Works closely with PACE IDT members and dispensing pharmacy to optimize PACE participant medication safety, compliance and adherence; attends daily PACE IDT meetings to collaborate with other PACE team members in reference to medication concerns or needs
  • Able to manage medications for chronic disease states, and keep up with clinical practice guidelines to provide evidence-based medication therapy recommendations to the providers.
  • Evaluates medication regimens in older adults considering co-morbidities, goals of care, preferences, and ability of elderly to adhere to medications. Familiar with Beers criteria, potentially inappropriate medications, polypharmacy, prescribing cascade, and the impact of medications on geriatric syndromes (e.g. falls, cognitive impairment).
  • Provides drug education to participants and other PACE team members as needed
  • Works closely with the PACE PBM to ensure optimal and compliant Prescription Drug Event claims processing and claims adjudication to CMS; reviews and completes PBM reports on an ongoing basis to ensure accurate and compliant PACE Part-D claims and billings to CMS.
  • Comfortable with prescription ordering platforms, systems and processes impacting the PACE Center and PACE Delivery Continuum to include PACE EHR, Pharmastar PBM interfaces, and other applicable PACE IT systems as appropriate
  • Able to supervise and train PACE Clinical Pharmacy Assistant, or intern/student pharmacists in daily functions to provide appropriate pharmacy services for participants
  • Able to independently perform above tasks and other clinical pharmacy tasks requested by supervisor without direct supervision or ongoing guidance
  • Prepared and administer vaccines to PACE participants permitted by NJ DOH, NJ BOP & recommended by CDC Guidelines
