About The Position

The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Requirements

  • PharmD Doctorate of Pharmacy Or Bachelor's Degree Bachelor of Pharmacy
  • Washington Pharmacist License (Upon Hire)
  • Basic Life Support (BLS) - where applicable

Nice To Haves

  • Acute care hospital pharmacy experience, 1 year or equivalent experience.
  • Completion of ASHP-accredited PGY1 residency.
  • Board of Pharmacy Preceptor License - when eligible and where applicable

Responsibilities

  • Provide comprehensive pharmaceutical patient care.
  • Follow department/ facility/regional policies, procedures and guidelines related to medication use system, pharmaceutical care and documentation of clinical interventions.
  • Communicate effectively in verbal and written form to key stakeholders (peers, caregivers, patients, providers).
  • Maintain knowledge of regulatory requirements and medication safety standards.
