This is a high-volume practice offering a rewarding, team-oriented work environment. The Clinical Pharmacist will be responsible for ensuring that the practice of pharmacy is in overall compliance with laws and regulations, and will provide professional pharmacy services as appropriate. They will assist in the development of appropriate clinical pharmacy goals and objectives, maintain up-to-date patient drug profiles, and provide patient medication counseling and education. The role involves implementing and providing all aspects of clinical pharmacy activities with an emphasis on therapeutic drug monitoring, clinical pathways, formulary management, pharmacoeconomics, and patient outcomes. The pharmacist will also perform Medicare Annual Wellness Visits, collaborate with physicians on medication use, respond to drug information requests, administer immunizations, and participate in Medication Therapy Management. Additionally, they will analyze scientific literature, educate healthcare professionals and patients, interpret pharmacokinetic data, establish cost-effective drug therapy programs, attend meetings, and utilize computer applications for work assignments. The role requires adherence to CVHS policies and procedures, and access to protected health information in accordance with guidelines. All staff will abide by accreditation standards in Mail Order Pharmacy and Specialty Pharmacy. Other incidental tasks may be performed as needed, and attendance as scheduled is required.
